16-Year-Old Girl Killed in Illinois Truck Crash
A teen girl was killed Tuesday evening in Montgomery, Illinois, when a semi trailer collided head-on with her van. Two other passengers in the van suffered minor injuries in the collision. The fatal truck accident occurred at the intersection of Orchard Road and Route 30 at around 8:20 p.m. Authorities say the 16-year-old girl was driving the Chevy van north on Orchard Road at the intersection with Route 30 when the semi trailer turned and slammed into the van head-on. The girl, whose name has not been released, was taken by helicopter to Good Samaritan Hospital in Downers Grove where she died Wednesday of her injuries. No citations were issued in the aftermath of the crash. According to Beacon-News, the investigation into Tuesday evening's tractor trailer crash is ongoing.

Tractor Trailer Driver Charged in Fatal Virginia Truck Crash
A tractor trailer slammed into two passenger vehicles Friday evening on Virginia’s Interstate 81, killing two people. The fatal truck crash occurred in Wythe County, Virginia at around 6 p.m. near mile marker 75. Authorities say a southbound tractor trailer crossed over the median and collided with a Ford F-150 pickup and a Dodge minivan. Two men were killed in the fatal truck crash. It was not immediately clear which car each victim was driving. Tractor trailer driver Charles B. Conner was charged with reckless driving and failure to maintain control of his rig, the Roanoke Times reported. Conner was uninjured in the crash. Authorities are investigating Friday evening's crash in Virginia.

Street Sweeper Runs Red Light, Killing Three
A distracted street sweeper driver has been cited for running a red light and colliding with a passenger vehicle on Sunday evening, killing three members of the same family. The fatal crash happened at the intersection of 171st Street and LaGrange Road in Orland Park, Illinois at around 8:30 p.m. Witnesses say the street sweeper was northbound on LaGrange when it collided with a southbound Toyota Camry as it was making a left turn onto 171st Street. Three people in the Camry -- 62-year-old Nazai Deis, his 49-year-old wife Wafieh Deis and their 15-year-old daughter Samah Deis -- were killed in the crash. The couple's 24-year-old daughter, who was driving the vehicle, survived the crash. Another vehicle was hit after the Deis's car was struck by the street sweeper, though the condition of that driver and vehicle are unknown. The Illinois State Police are currently awaiting the results of blood and urine tests to determine if the driver of the street sweeper was under the influence of drugs or alcohol at the time of the tragic crash. Authorities are not releasing the name of the street sweeper, pending further investigation.

Two Killed After Box Truck Causes Multiple Vehicle Crash
Two people were killed Friday afternoon after a box truck collided with multiple vehicles in Gwinnett County, Georgia. The fatal truck crash occurred on Interstate 985 near Georgia Highway 20 at around 3:30 p.m. According to authorities, traffic was slowed or stopped on the southbound side of the Interstate when a box truck rear-ended a Nissan passenger vehicle, killing both occupants inside. The box truck, with the Nissan still attached to it, then collided with a Dodge pickup truck and a minivan towing a trailer. The driver of the Dodge pickup was taken to an area hospital with injuries that are not considered life threatening. The identities of the two people killed have not yet been released. At this time, authorities are uncertain what caused the deadly crash. An investigation is currently ongoing.

Big Rig Driver Cited For Multiple Vehicle Crash
A big rig viciously rear-ended two passenger vehicles Tuesday afternoon in Perrysburg, Ohio, sending one woman to the hospital with serious injuries. The big rig crash occurred on Interstate 75 near the exit for U.S. 20 at approximately 12:20 p.m. Authorities say truck driver Buck Fordney rear ended a car driven by Diane Lambert. Lambert's car then collided with another vehicle before ending up in a ditch on the side of the Interstate. Fordney was taken to a local hospital with non-life-threatening injuries. Lambert, who had to be extricated from her vehicle by the Perrysburg Township Fire Department, was rushed to Mercy St. Vincent Medical Center in serious condition. Mr. Fordney was cited by law enforcement for failing to keep adequate distance in front of his rig. An investigation into Tuesday's big rig crash is ongoing.

One Dead, Two Seriously Injured in Fatal Big Rig Crash
A man was killed Friday morning when his car was viciously rear ended by an out-of-control tractor trailer. The fatal crash occurred on Interstate 10 in Indio, California at approximately 8 a.m. Authorities say 50-year-old tractor trailer driver Jaswinder Singh slammed on his brakes, which forced another big rig, driven by 52-year-old Howard Sands, to swerve into the left lanes of I-10. The crash caused a chain-reaction which involved three passenger vehicles. The driver of a passenger car was killed in the multiple vehicle crash. Howard Sands and the driver of another passenger vehicle involved in the wreck were flown by helicopter to area hospitals with moderate to major injuries. At this time, authorities are uncertain what caused the multiple vehicle accident. An investigation is ongoing.

Family of Burbank, California Motorcyclist Killed in Truck Crash Files
Wrongful Death Lawsuit against Florida-based JCI Jones Chemicals
The family of Galo Ulloa, killed in a Burbank, California tractor trailer crash, filed a wrongful death lawsuit on Monday March 6, 2012, case number CV12-1889-CBM (MANx), in the U.S. District Court for the Central District of California in Los Angeles against Sarasota, Florida-based JCI Jones Chemicals, Inc.
The complaint, filed by the Los Angeles-based law firm of Baum, Hedlund, Aristei & Goldman on behalf of Galo’s oldest son and representative of his estate, Dave Ulloa, alleges that JCI Jones Chemicals, Inc. was negligent, careless and reckless in operating the tractor trailer that caused the fatal crash. According to the lawsuit, JCI breached its duty to use due care in operating the truck and in obeying applicable laws related to the safe and lawful operation of a commercial motor vehicle transporting goods, including hazardous materials.

Three People Forced to Leap From Overpass Due to Passing Tractor Trailer
Two women and a crash victim they were trying to assist are in the hospital after the three were forced to jump off an overpass in an effort to dodge a speeding big rig. The incident occurred Monday morning on Interstate 79 in Perry Township, Pennsylvania. Authorities say the two women, who are both college students from Waynesburg University, stopped to help a man that fell asleep behind the wheel and crashed his car. It was during this time that a semi trailer passed dangerously close to the three of them as they were standing on the side of the road, forcing them to jump off the overpass or be hit by the oncoming truck. The three were taken to Ruby Memorial Hospital after falling an estimated 40 feet. One of the women is in critical condition and the other two are in serious condition. Authorities are currently investigating the incident.

One Child Dead in Crash Between Dump Truck and School Bus
A dump truck collided with a school bus filled with students from Chesterfield Elementary School on Thursday morning, killing one child and injuring 17 others. The fatal crash occurred at the intersection of Old York Road and Bordentown Chesterfield Road (Route 528) in Chesterfield, New Jersey. No charges have been filed yet as authorities are currently piecing together what caused the fatal accident. Three of the children injured in the crash had to be airlifted to Cooper University Medical Center with life-threatening injuries. The drivers of both the school bus and the dump truck were taken to area hospitals but their injuries were not serious. The child that was killed, an 11-year-old girl, is the daughter of a New Jersey state police trooper who may have had at least one other child on the bus. The investigation into Thursday's fatal crash in Chesterfield is ongoing.

30-Year-Old Man Killed After Being Rear-Ended by Tractor Trailer
A tractor trailer viciously rear-ended a passenger van filled with oil field workers on Wednesday morning killing one of its passengers. The fatal crash occurred on Highway 59 near Victoria, Texas. Officials say a tractor trailer cut in front of the passenger van, causing the van to stop before it was rear ended by another tractor trailer. Several passengers were trapped in the wrecked van before being rescued by emergency responders. Robert Lee Garcia, a passenger in the van, was pronounced dead at the scene of the accident. Authorities are investigating Wednesday morning's truck crash near Victoria.

Truck Driver Charged With Vehicular Manslaughter Pleads Not Guilty
Byrus Lyndon Gatewood, a truck driver charged with crashing his rig into multiple vehicles stopped in a construction zone, has pleaded not guilty to vehicular manslaughter charges. Gatewood is facing charges for killing 62-year-old Dayle Fowler, who was standing outside her vehicle while traffic was stopped in a construction zone near Jerome, Idaho. Prosecutors say Gatewood was driving his truck 64 miles per hour in a construction zone with a posted speed limit of 45 miles per hour when the fatal crash occurred. According to Times-News in Magic Valley, Gatewood entered his plea on Monday in Jerome County's 5th District Court. Trial is set to begin May 16.

Tractor Trailer Crashes in Work Zone Killing One and Injuring Three
The driver of a tractor trailer crashed into a work zone on the Ohio Turnpike on Tuesday morning, killing a maintenance worker and seriously injuring two others. The tractor trailer driver was also injured in the fatal crash. Authorities say the maintenance workers were repairing a bridge 30 miles outside of Toledo when the tractor trailer drifted into the work zone and hit three workers. One of the workers, 53-year-old Forest Fletcher, was killed in the crash. Two other maintenance workers and the tractor trailer driver had to be airlifted to local hospitals with serious injuries. At present, no charges have been filed against the tractor trailer driver. An investigation into the cause of Tuesday's accident is ongoing.

Young Man Critically Injured After Semi Runs Red Light
A 21-year-old man is in critical condition after his pickup truck was sideswiped by a semi Monday morning in Chippewa Township, Pennsylvania. The crash occurred on Route 51 at around 8:15 a.m. Authorities say 21-year-old Jordan Agnello was turning onto Route 51 from McKinley Road when his pickup truck was hit by a southbound semi. Agnello was airlifted to a hospital in Pittsburgh following the collision. All indications point to the semi running a red light at the intersection, authorities said. The semi is owned by PGT Trucking out of Potter Township, Pennsylvania. No charges have been filed against the semi driver, as the investigation into the cause of Monday's crash is still ongoing.

Tow Truck Driver Struck and Killed by Big Rig
A tow truck driver attempting to load a pickup truck onto a flatbed was struck and killed by a big rig just after midnight on Wednesday in Lemoyne, Pennsylvania. The fatal accident occurred on Route 581 near the Interstate 83 split. State Troopers say tow truck driver Andrew Smith, 23, died at the scene. The 63-year-old big rig driver, who works for New England Motor Freight, was uninjured in the accident. At present, no charges have been filed against the big rig driver. An investigation into the cause of the fatal accident is ongoing.
