A twin engine Cessna 402, operated by Adventure Airlines, crashed as it took off from the Grand Canyon West Airport in Arizona on June 19, 1992. There were 10 people aboard the aircraft. No one survived the crash.
Shortly after take-off, the Cessna lost power in its right engine. According to witnesses, the aircraft made a steep right turn that progressed into a dive. The pilot had transmitted that he was experiencing a problem moments before the aircraft dove to the ground.
Aviation lawyers from Baum Hedlund represented two passengers in this crash.
The National Transportation Safety Board (NTSB) investigated the crash and concluded that the cause was pilot error. By not following the emergency procedure of a failed engine on takeoff, and by not taking control of the aircraft speed, the flight was doomed. The NTSB also blamed the airline for contributing to the crash by not properly training the pilot to handle the emergency situation in an aircraft with a dense load and operating in a high-elevation airport.
